Responsibilities:
- Provides consultation and case management services to clients, which includes development and implementation of a comprehensive service plan based on assessment results, determines the appropriate level of care and service provider based on established policy, updates and conducts follow-up and monitors client service plan as needed.
Coordinates scheduling of evaluations and completion of screening paperwork and referral packets.
Coordinates and reviews case records, documents and information prepared by staff for adherence to agency policies and procedures.
Develops and implements a training program for case management staff.
Interprets rules, regulations, policies, standards and objectives of the case management program to subordinates and others.
Prepares reports, correspondence, social studies and keeps necessary records.
Conducts case management conferences and staff meetings.
Serves as liaison and provides outreach to appropriate groups and individuals.
Reviews and approves the determination of the level of care by subordinate case managers.
Reviews and approves the determination of funding arrangements and the authorization and recommendation of services for alcoholic and other drug clients, in conjunction with the D &A Administrator.
Supervises reviews and approves the development and implementation of a comprehensive service plan based on the results obtained by the case manager from the assessment process.
Reviews and approves updates to the client service plan as made by the client's case manager.
Supervises the coordination and provision of access to available alcohol and other drug ancillary services.
Oversees the monitoring of client treatment progress and the performance of utilization review activity, such as case manager contacts with treatment providers, continued stay reviews.
Reviews case notes to ensure proper billing occurs.
Performs functions of organizational planning, interdepartmental coordination, public relations, and liaison work.
Maintains liaison with referral agencies, such as, but not limited to, school district SAP Teams, probation and parole, CYS.
Liaison to Plans of Safe Care Team.
Oversight of the SOR Program funding and assure completion of GPRA assessments initially, at 6 months, and at discharge.
Approves subordinates' leaves and timesheets, completes performance evaluations, and recommends disciplinary actions when needed.
Participates in hiring and training process for new case managers.
Supports Administrator with all aspects of drug and alcohol programming, planning, and local service delivery, including development of annual plans and reports.
Completes reports and surveys as required.
Attends local, regional, and statewide DDAP and other professional organization meetings as beneficial to SCA management.
Responsible for quality management activities to ensure compliance with all applicable State, Federal and funder requirements.
Attends and participates in Forest-Warren Human Services' management and supervisor team meetings and projects/assignments as directed by the Human Services Director-MH/ID Administrator.
Performs other related work as requested/required.
Crosses program lines when the need arises.
Mandated Reporter.
Work toward a trauma informed, coordinated, and collaborative system of services and support driven by the youth, adults, and families it serves.

The check will bounce and the victim is left responsible.
